This service is for employers and individuals with approved roles to retrieve copies of their Employment Tax Returns (94x family of forms). Use by any other entities is prohibited. You can access your current and previous year's submissions only. To use this service your computer or mobile device needs access to a camera.

When you prepare your employment form, you are required to keep a copy before submitting and paying for it. Our system is designed so that you must download your form before you can submit it for processing. The default location of your return will generally be in your DOWNLOADS folder.

What You Need

A minimum of one employment tax return 94x submitted through our website.

Your registered image using our Face Recognition Utility (FRU). To register, use the AutoFill button located within the specific type of form you submitted.

What You Get

A list of all employment forms submitted during the current and prior year only.
